Evaluation: $200 (up to 2 hours)
Treatment Session: $100 (up to 1.5 hours)
Phone Consultation: $25 (up to 30 mins)

Combination packages:
Evaluation+ 4 treatment visits= $500(saves $100)
Evaluation+ 3 treatment visits= $425 (saves $75)
Evaluation+ 2 treatment visits=$350 (saves $50)
Missed Visits/Cancellations: $25

Evaluations will include review of medical history, initial assessment of function and performance, and home environment. Education as well as treatment may begin on evaluation, time allowing. 

*We currently do not accept health insurance policies for reimbursement for our services. We are also unable to accept Medicare or Medicaid beneficiaries due to Medicare rules and regulations.*
